In managed mode (the default) LazyApp generates, hashes, sends and later verifies the code. In delivery_only mode you supply code and verify it yourself.
Codes are never stored in plaintext and are redacted from logs and webhooks.
Refusals carry a code you can branch on. Waiting is the answer for otp_resend_too_soon, otp_client_ip_limit and a locked number, so those are 429 with Retry-After. Sending and verifying have separate per-key rate limits.

Headers
Optional. Makes this write safe to retry: the same key replays the first response instead of acting twice.
255Body
32Defaults to managed.
managed, delivery_only Required for delivery_only.
4 - 10^\d+$An approved authentication template. Defaults to the workspace's configured OTP template. Without one, a code can only go as plain text while the recipient has messaged you in the last 24 hours; otherwise the request is refused with otp_template_required.
12864The template language. Defaults to the template's only language in this workspace, and otherwise to the configured default.
15^[a-z]{2,3}(_[A-Za-z]{2,4})?$"en"
Your end user's IP address. When given, one address may request codes for a limited number of different numbers an hour. Without it that rule is skipped, so a server sending every sign-in from one address is not capped.
